SUBSTANCE: endoprosthesis has hollow, head, neck, leg, washer consisting of movable members, separated by gaps one from another; washer is disposed between surface of head and bottom of hollow. Endoprosthesis also has corrugated casing made of resilient material, which casing seals cavity of joint, which cavity is filled with lubricant. Members of washer are made in form of rings, which rings are put one onto another by their flat surfaces making contact one to another by means of surfaces being parallel to edge of hollow. External surfaces of rings are congruent to internal surface of hollow, internal surfaces are congruent to surface of head. Member of washer disposed between poles of head and of hollow is made in form of concave-convex menisci with spherical surfaces disposed concentrically. Top convex surface is congruent to internal surface of hollow and lower one, being concave in center, namely, head; flat edges of meniscus are conjugated with flat ends of ring-shaped member disposed lower. Gaps provide ability of making rocking shifts of members of washer between surfaces of head and of hollow, one relatively other, as well as rotational motions around their own axes. Members of washer and head are sustained in hollow by means if flange with fold, which flange is fixed at edge of hollow by means of air-proof washer. Corrugated casing is attached along end of fold and onto plug of neck. Cavity of casing, filled with lubricant is connected with cavity of joint, formed by capillary spaces, through openings in fold of flange.
After placing the cup, the same approach is applied to the preparation of the femur (upper leg bone). Firstly, the central bone canal is widened to the required size, then the femoral part of endoprosthesis (stem) is fastened. With cement-free endoprosthesis, the stem is driven into the femur, while with the cement stem, the stem is used the same way as for the cup. After implanting the stem, the head is placed on it, and the parts are repositioned (the head is placed back into the cup). All three components of endoprosthesis (head, stem and cup) are manufactures in several sizes, and are individually adjusted to every patient. Thus, the anatomy of the hip is reconstructed and the length of legs evened, as they are often uneven before the operation. After checking the movement of the joint and its stability, the operative field is closed by stitching the joint capsule, then the musculature and the final subskin, by special intracutaneous stitches located under the skin level, which cannot be seen. Such stitches do not need to be removed, and they are aesthetically most acceptable owing to the thin scar they make which eventually fades.

In our institution, classic implant of complete hip endoprosthesis lasts between 60 and 90 minutes. With cement-free endoprosthesis, the cup is thrust into the pelvis and may be additionally fixed with special screws. With cement endoprosthesis, the cement is used as a means for fastening the cup to the acetabulum bone.

SUBSTANCE: endoprosthesis of hip joint contains head, neck, pelvic component, cylinder-shaped leg tapered off conically and muffled in the distal part. The cylinder-shaped trochanteric part with the centrally fixed rod is installed in the proximal leg part so as to enable sliding along its internal surface.
