Surgery Part I: Intraocular Prosthesis.

The method of surgical treatment of glaucoma, which consists in cutting out the superficial scleral flap, cutting out from the underlying layers of the sclera of the second deep scleral flap, prior to the exposure surface of the ciliary body, the opening angle per what about the flap interrupted sutures, characterized in that in the anterior chamber of the eye, under the superficial scleral flap and on the surface of the sclera in the area of antiglaucomatous operations impose additional pharmaceutical composition comprising a non-steroidal anti-inflammatory agent, a corticosteroid, hemostatic and viscoelastic in the following these components, wt.

as a means of immediately anchoring the prosthesis to the eye during surgery.

The telescope prosthesis is surgically implanted in the posterior chamber of the eye, in the capsular bag after removal of the natural lens. It is held in position by haptic loops.

Excessive scarring of the filtration zone.The objective of the method is to eliminate the possible complications associated with inflammatory reaction, bleeding from vessels in the area of operation, hyperfiltration intraocular fluid, as well as the prolongation of the hypotensive effect of antiglaucoma surgery
The technical result that is achievable with the use of the invention is the reduction of the inflammatory response, reducing the incidence of complications associated with bleeding in the area of operation, the formation of an adequate resistance to the outflow of intraocular fluid for the prevention of complications associated with hyperfiltration in the technical result is achieved by in the method of surgical treatment of glaucoma, including cutting out the superficial scleral flap, cutting out from the underlying layers of the sclera of the second deep scleral flap to the exposure surface of the ciliary body, the opening angle of the anterior chamber, conducting the basal iridectomy, excision of the deep flap of sclera and the fixation surface of the flap interrupted sutures, according to the invention in the anterior chamber of the eye, and (or) under the superficial scleral flap and / or on its surface, and (or) on the intact surface of the sclera in the area of antiglaucomatous operations under the mucous membrane of the eye impose additional pharmaceutical composition comprising a non-steroidal anti-inflammatory agent, a corticosteroid, hemostatic and viscoelastic in the following these components, wt.%:
Nonsteroidal anti-inflammatory component is 0.01 to 1.0
Corticosteroid - 0,01 - 1,0
Hemostatic - 0,5 - 10,0
Viscoelastic - Rest
The pharmaceutical composition as non-steroidal anti-inflammatory component may contain diclofenac sodium from 0.01 to 1.0 wt.%.The pharmaceutical composition as corticosteroid may contain dexamethasone 0,01 is as hemostatic may contain etamzilat sodium 0.5 to 10.0 wt.%, and/or aminocaproic acid 0.5 to 10.0 wt.%, and/or calcium gluconate 0.5 to 10.0 wt.%.The pharmaceutical composition as viscoelastic may contain 0.1 to 3.0 percent of sodium hyaluronate, and/or 0.1-6.0% chondroitin sulfate, and/or 0.1-6.0% of water-soluble cellulose derivatives in isotonic saline solution at pH 7,0-7,5.It should be noted that viscoelastic using the proposed method starts to play a role depot for accumulation of the drugs included in the pharmaceutical composition.

The most common risk factors include diabetes mellitus (DM), long-term corticosteroid (topical, systemic, intravitreal, inhaled or oral) use and history of prior intraocular surgeries (AAO, 2016).

Prosthesis Surgery (Evisceration with Intraocular Prosthesis) Contact Lens Placement;

An Intraocular Prosthesis is used to relieve discomfort in an eye that is irreversibly blind. is the most common condition which is treated in this way. The surgery involves an incision in the eye though which the inner contents of the eye (lens, retina, iris, ciliary body) are removed. A prosthetic ball is then implanted into the eye to maintain it’s shape. Once healing is complete, the eye looks nearly normal. The eye blinks and moves in coordination with the other eye. It is important to recognize that the outer layers of the eye are still living tissue and, thus, subject to injury or infection although this is rarely a problem.

Dylan is a Jack Russell terrier affected with glaucoma in both eyes. Unfortunately, the glaucoma resulted in irreversible blindness and discomfort. Intraocular prostheses were implanted to relieve the pressure and discomfort while retaining a nearly normal appearance to the eyes. Dylan’s “Mom”; reports: “Dylan feels better and has totally adjusted to being sightless. He no longer has the pain of eye pressure and does not have to have expensive drops three times a day.”
